your first visit
Please arrive 10-15 minutes early to your appointment to allow for check-in and treat yourself to a beverage. We provide complimentary toothbrushes if you’d like to brush before your appointment.
Please allow 2 hours for your new patient exam. Our new patient exams are comprehensive in nature, and there’s simply no expedited way to provide this high level of care. Dr. Zarrabi devotes his time and expertise to patient education. You will leave your appointment knowing what is going on in your mouth, why, and what you can do about it. Additionally, your treatment plan will be presented in a very clear, transparent way, so you can easily anticipate your costs.
canceling/rescheduling an appointment
Please note that your appointments are reserved exclusively for you; therefore, it is very important that you be on time and show up to your appointments. If you need to cancel or reschedule, please give us the courtesy of doing so at least one full business day in advance. (This means that if you cancel an 11:00am Monday appointment, you need to have done so by the end of the business day that previous Thursday.) Please keep in mind that a broken appointment could have been utilized by another patient in need of care.
There is a cancellation/rescheduling fee for not canceling/rescheduling at least one full business day in advance.
We accept most major PPO plans, including the following, but PLEASE CALL TO VERIFY whether or not we are IN NETWORK with your particular plan:
Aetna, AIG, Anthem BCBS, Assurant, Ameritas, BCBS TX, Cigna, Dental Select, DNOA (Dental Network of America), Guardian, Humana, Principal, United Concordia, and UnitedHealthcare.
Please note that we are not accepting NEW Metlife patients that wish to see Dr. Zarrabi as an in-network provider, as on May 1, 2018, we will no longer be a Metlife Preferred Provider. We will continue to see and accept Metlife patients but as an out-of-network provider.
We accept personal and certified checks, Visa, Mastercard, Discover, and American Express.
We also offer patient-financing through CareCredit.
We DO NOT accept DMOs/HMOs.
